I have tried to translate as best as I could.

waroop digambar aajanubahu bhavyakaya natha – Divyakaya natha ll Dhru, ll
Hrudaya niranjani sudh premdhruti bhavachya vaati ll
Bhajanand prakash deyuni ujal laya jyoti ll 1 ll
Sarvaswaparna naivadhashi thevilechi pudti l
Sanmti,saddhruti,satkruti sadgati Prasad dhya haati ll 2 ll
